Cancellation Policy

Eligibility: Age Range: Youth aged 6-18 years old are eligible to join iThrive programs. Residency: Open to all youth regardless of residency, with a focus on serving the local community. Health and Safety Information: Medical Conditions: Disclosure of any relevant medical conditions, allergies, or special needs. Medications: Details of any medications the youth is currently taking. Doctor’s Information: Contact information of the youth's primary care physician. Insurance Information: Health insurance provider and policy number (if applicable). Program Interests: Preferred Programs: Indicate interest in specific programs (e.g., Kids Thrive, Tweens Thrive, Teens Thrive). Activity Preferences: Select preferred activities (e.g., arts and crafts, sports, STEM, leadership development). Membership Agreement: Code of Conduct: Agreement to follow iThrive’s code of conduct, which includes respect for others, participation in activities, and adherence to safety guidelines. Photo Release: Consent for iThrive to use photographs or videos taken during programs for promotional purposes. Liability Waiver: Acknowledgment of the risks associated with participation in iThrive activities and agreement to waive liability. Privacy Policy: Agreement to iThrive’s privacy policy regarding the use and protection of personal information. Payment Information: Membership Fees: Subject to change Payment Method: Credit Card, Debit Card, or approved electronic payment methods Recurring Payment Authorization: Consent to recurring monthly payments for the membership fees Registration Process: Complete Online Registration Form: Accessible through the iThrive website. Submit Required Documents: Upload any necessary documents (e.g., health records, photo release form). Review and Confirmation: iThrive staff will review the registration and contact the parent/guardian for any additional information or clarification. Payment Processing: Ensure the membership fee is successfully processed. Welcome Package: Once registration is complete, a welcome package with program details and schedules will be sent to the youth and their parent/guardian.
